How Forms Look
1. New Lead Form
The new lead form will prompt for the fields you want to collect as well as any consents you need

The form will show the progress at the top and go through the questions

Depending on how they answer, we will show different ends! The first example show unqualified, second shows route to campaign, and third a default end.
